Address/Emergency Contact Change Form

  1. Please complete the information below to notify Human Resources and Payroll of a change of address, phone number, or to change your emergency contact information.

    Police Officers and Firefighters: Please note you must also complete a PSPRS change of address form.

    All other employees, please visit to change your address.

    For name changes, please stop by the Human Resources office as there are multiple forms to be completed. You will need to bring a Social Security card with your new name; other documentation may be required as well (marriage certificate, etc).
